Cholera

The acute diarrheal disease caused by Vibrio cholerae that spread in seven global pandemics from 1817 to the present, killing millions and driving the sanitary reforms that created modern public health infrastructure. Cholera is the founding disease of epidemiology: John Snow's 1854 investigation of a London outbreak established the field's core methods before germ theory existed, demonstrating that disease patterns could reveal causation through spatial analysis alone.

In 1854, physician John Snow mapped every cholera death on a street in Soho, London, onto a single piece of paper. The deaths clustered around one water pump on Broad Street. Snow persuaded the local authorities to remove the pump handle. The deaths stopped. He had no germ theory. He had no microscope capable of seeing the bacteria. He had a map, a hypothesis about contaminated water, and the authority to act on it. That act — a structural intervention in a shared environment rather than treatment of individual patients — is the founding moment of modern public health. Cholera made it possible by killing fast enough, and leaving clear enough patterns, to see what no one had seen before.
